Session Rates for Services Provided

PRIVATE PAY

Individual adult, couples, or family therapy, by appointment, is $175.00 for a 50-minute, therapeutic hour. We accept cash, personal cheques, and all credit/debit cards (a $5.00 flat fee is assessed with the use of each credit/debit card).
 
Employee Assistance Program (EAP)
EAP is a confidential, short-term, assess and refer program. There is no-cost to you as your employer typically pays for the program. Please contact your employer’s website or Human Resources Department for details. Most EAPs offer sessions for the employee or family member to meet with a licensed professional to discuss your situation, personal or work-related, and be provided with referrals including community resources. When requesting services, please ask your EAP if Andrea Keith LMFT CEAP is in their provider network. If not, you may ask that Andrea be added.
 
Insurance Procedures
Andrea Keith LMFT CEAP is an in-network provider with the following medical insurance companies:
     Anthem Blue Cross, Beacon Health Options formerly Value Options, Blue Shield of California Administered by Magellan, HealthNet, United Behavioral Health (Optum), United HealthCare (Optum). A copy/photocopy of the front and back of your insurance card, your date of birth and address is required for preapproval.
 
Pet Loss Support Group
This is a community Support Group specific to Pet Loss that meets once monthly, on a Saturday (first or second), from 12:30 pm - 1:30 pm.
Although we do not require RSVPs, and walk-ins are welcome, we encourage you to please confirm the meeting date prior to arrival.
Please refer to our Pet Loss Support Group schedule listed below. We request a $25.00 fee however, no one is turned away due to lack of funds.
Put what you can afford into an envelop with your first name in the "from" area, and Andrea's name in the "to" area of the envelope. Hand in when payment is collected.
No one else will know what amount you paid and reduce the confusion that may occur otherwise.

All payments are appreciated and cover overhead and Andrea's time.
You may "pay it forward" to cover the cost for someone else that cannot afford it. Please share your intentions with us.
Some insurance plans are accepted but must be preapproved prior to the support group meeting.
 
Pet Loss counseling sessions are also available for individuals, couples, and/or the family by appointment. Please inquire.
 
ANGER MANAGEMENT CLASSES
  • This Program meets Court requirements as it was O.C. Dept of Probation-approved in 2012.
  • Classes are for adults 18 years-of-age and over.
  • Perfect for Court-ordered, employer-mandated, and attorney-suggested individuals.
  • Voluntary participants are welcome and regularly attend for self-improvement and to learn better coping skills.
  • Classes are open to both men and women, LGBTQAI-friendly/affirming.
  • A one-time enrollment appointment is required prior to attending. No walk-ins. Appointments must be scheduled in advance and the cost is $75.00.
  • Each Anger Management class is $30.00. Classes are based on a 10-lesson curriculum.
  • Once enrolled, you may attend when your schedule allows.
  • Graduates are eligible to continue to attend classes at no additional fee.
  • Classes are offered:  Tue 5:30 pm - 7:00 pm,  Wed 5:30 pm - 7:00 pm, and Thur 11:00 am - 12:30 pm.
  • Most insurance will not reimburse for anger management classes as anger is a normal emotion on par with sad, happy, and fear.

ENROLLMENT PROCEDURES

AN APPOINTMENT IS REQUIRED TO ENROLL

An enrollment appointment must be scheduled, in advance and prior to attending classes. No walk-in appointments are accepted. To reserve your enrollment appointment, prepayment is required and receipt must be confirmed, in advance, of your enrollment appointment.

If you find, for any reason, you must change, reschedule, or cancel your enrollment appointment, please inform us promptly. We do maintain a 24-hour cancellation policy. Details on this are provided below.

COMMUNICATION and CONTACT

Our office must maintain a reliable method of contacting you. Therefore, we will request your telephone number, texting options, and email address. If you do not choose to communicate through text messaging please let us know your preferences.

REQUIRED ENROLLMENT DOCUMENTS

Once you have scheduled your enrollment appointment, you will be emailed two packets of blank forms. You must print, complete in full, and bring these required documents with you for your enrollment appointment. Hard copies are necessary as some of the forms require signatures. Without these completed documents, your enrollment appointment will be incomplete and you will be required to schedule a second enrollment appointment and incur an additional $75.00 enrollment fee.

ENROLLMENT FEE

An enrollment fee in the amount of $75.00 USD is required as payment and nonrefundable. This enrollment appointment is not to be considered a therapy appointment and is not reimbursable by most insurance companies. The purpose of this enrollment fee is to cover the Administrator’s time in which to meet, answer questions, and provide an orientation to the Anger Management classes, structure, location, and available amenities including free parking, availability of water, coffee, tea, and restrooms.

PREPAYMENT IS MANDATORY

The cost of the enrollment appointment is $75.00. Prepayment of this fee is required to reserve your scheduled appointment. Once your payment is received by our office, we will then confirm payment receipt, with you. If prepayment has not been received and your appointment is scheduled within two (2) days, we will contact you to confirm your continued interest in enrolling. There are no fees to reschedule your appointment at this time as we may offer that appointment time to someone else.

HOW TO PREPAY

You may prepay in the following ways:

1.    Mailing or dropping off a check, made out to Andrea Keith LMFT CEAP at the office located at: 4199 Campus Drive, Suite 550, Irvine, CA 92612

2.    Requesting and receiving an emailed invoice, from us, through Square. A $5.00 processing fee will be assessed with each credit and/or debit card transaction.         *** Important: Please note, we do not store or keep any credit card information on file.

PAYMENT CONFIRMATION

Following receipt of your prepayment, we will confirm, with you, that your prepayment has been received. In addition, you will receive a receipt for payment at the conclusion of your enrollment appointment.

YOUR APPOINTMENT CONFIRMATION IS REQUIRED

If your prepayment has been confirmed as received, one (1) day prior to your scheduled appointment, you will receive a text message from our office requesting you to confirm that you will be in attendance for your scheduled enrollment appointment. This confirmation also includes your commitment to have your enrollment paperwork completed and ready to submit. It is imperative that you respond before 5:00 pm that same day. If we have not received your appointment confirmation by 5:00 pm that day, we reserve the right to offer that appointment time to someone else. You will be required to reschedule your enrollment appointment for another date and time.

If you do attend your enrollment appointment and your paperwork is not completed and ready for submission, you will be required to reschedule a second enrollment appointment and, again, prepay the enrollment fee of $75.00.

CANCELATION POLICY

If, for any reason, you should need to change your appointment, there will be no charge, provided you give us at least a 24-hour notice to avoid a $40.00 late cancellation fee. If we are not given at least 24-hours notification of your intent to cancel, the $40.00 late cancellation fee will be deducted from your $75.00 prepayment. To schedule a follow up appointment to complete your enrollment, prepayment of $40.00 will be necessary to reserve your appointment.

Relationship Boot Camp: Recalibrating Codependent Behavior
  • This is a structured four to six-week workshop for adults 18 years-of-age-and-over dealing with issues of codependency.
  • A one-time enrollment appointment is required prior to attending any Boot Camp sessions. The cost to enroll is $75.00.
  • Some insurance is accepted but must be preapproved prior to enrollment and attending classes.
  • Each workshop class is $30.00.  This is based on a four (4) or six (6) lesson class curriculum. The length of the workshop will be discussed at enrollment.
  • Graduates are then eligible to attend an on-going monthly process group for $30 each session.
 
Gambling causing any problems? Treatment is available for the individual, couple, and/or family
  • No-cost treatment of up to five (5) sessions is available for adults; 18 years-of-age and over, problem gambler or loved one of a problem gambler.
  • Treatment is also available for family members, loved ones, friends, or any adult affected by a Problem Gambler's behavior.
  • Treatment is available for the Problem Gambler. All sessions are confidential.
